New patient form is downloadable from the footer of any page. To save your time, download the form and fill them out and bring with your first appointment. Please arriave 10-15 minutes early if you choose to work on the form in our office.

What to Expect During your first visit
Your acupuncture session will typically begin with an initial 5-10 minutes consultation, followed by an acupuncture treatment. The consultation includes minimal paperwork with some in-depth questions. The Doctors will then meet with you and answer any questions you may have at that time. The goal of the consultation is to get to the root of your problem. It involves a tongue and pulse diagnostic assessment to assess your organs and overall health in addition to your primary concenrns. Our Doctors will then prescribe an individualized treatment plan designed specifically for you. The treatment session that usually last about 40-60 minutes. Your first session, along with most subsequent sessions will and may include acupuncture and other TCM methods such as cupping, electro-acupuncture, etc., depending on the Doctors' determinations and findings at the time of your visit.
